Nurse Practitioner

Job Description
We are seeking a dedicated and experienced Nurse Practitioner to join our dynamic team. Reporting to the Chief Medical Officer and working closely with our healthcare professionals, you will be responsible for providing high-quality mental health care, including assessment, diagnosis, treatment planning, and management of patients. Your role will involve collaborating with psychiatrists, psychologists, and other team members to ensure comprehensive patient care.

Responsibilities

• Conduct comprehensive mental health assessments and evaluations.
• Develop and implement individualized treatment plans.
• Prescribe medications and monitor their effectiveness.
• Provide psychotherapy and counseling as appropriate.
• Collaborate with multidisciplinary team members to coordinate care.
• Educate patients and families about mental health conditions and treatment options.
• Maintain accurate and timely documentation in electronic medical records.
• Participate in quality improvement initiatives and ongoing professional development.

Qualifications

• Master’s or Doctoral degree in Nursing from an accredited institution.
• Current Nurse Practitioner license in Ontario.
• Certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP) preferred but not mandatory
• Minimum of 1 years of experience in mental health care.
• Proficiency in electronic medical records systems.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively in a multidisciplinary team environment.
• Commitment to patient-centered care and continuous learning.
